How to Register with the Surgery

NEW PATIENTS REGISTERING WITH THE PRACTICE

We welcome new patients to our list, application forms are available from reception.  We do ask all patients to bring along some form of identification and proof of UK residency - this helps us to determine your eligibility for free NHS care.  All new patients are offered the opportunity to have a new patient health check.  This is your choice and will enable us to find out important information about you.  However, your registration is not dependent upon this check being carried out

Things we need to know about you:

  • Medications: We ask all new patients to bring along details of current medications, e.g. prescription slips or empty boxes.  This is particularly important for patients who are from overseas and registering with the NHS for the first time.
     
  • Immunisations: Please ensure that we have up to date details of any immunisations that you have had.  We ask parents to bring along the details of their children's immunisations.
